We are currently open for a limited number of new registrations.
You are welcome if you live in our care area (Stadsdriehoek) and do not yet have a GP.
You can see whether you live in our service area on the map at the start of the registration form.
A valid Dutch basic health insurance or specific international policy is also required.
What do you need?
- A valid ID.
- Your citizen service number (BSN).
- Your health insurance card or policy details.
- The details of your previous GP, if you had one.
What happens next?
Register today? Your registration will be processed by Tuesday 6 October at the latest.
- You will receive a message from us once your registration has been processed.
- After that, we will explain how the patient portal works.
- We will request your file from your previous GP; you do not need to do anything for this.
